BACKGROUND Living with dementia entails changes in the levels and locations of care. At these transitions in
care people with dementia and their families are at risk of reduced well-being. Yet we have relatively little
research on how best to improve them.
AIM We seek to establish a unique, cross-institution, multi-disciplinary DTC to transform transitions for people
affected by dementia, drawing on University strengths in person-centred care, pharmacy and patient safety. This
unique combination of perspectives on transitions in terms of both locations and levels of care will focus on wellbeing,
increasing carer knowledge and understanding, improving professional communication and behaviour,
and care processes and systems.
METHOD Students will use a range of literature reviews and qualitative and quantitative approaches to codesign
and, where appropriate, conduct feasibility tests of theoretically informed complex interventions.
Supervision will be provided by a multi-disciplinary team of senior and junior academics, and the work enhanced
by a panel of international experts, supported by PPI and practitioner panels.
OUTCOMES With matched funding of 3 studentships, this research programme will identify approaches to
improving transitions for people affected by dementia, the most promising of which, with our students, we will
apply for external funding to rigorously test.
