Lay summary
Lay summary: Dementia is a chronic condition with major consequences for the patient, their families and society. The possibilities for the welfare and quality of life of people with dementia and their families are not sufficiently used to improve the current reactive and fragmented care.
This transition-experiment ‘integrated care’ dementia care supports people with dementia and their carers, tailored to their individual needs, integrated, and at the right time.
Sub goals are: Better support for informal carers; gain insight into facilitating and impeding factors for integrated dementia care; designing and testing an online record for data-sharing on integrated dementia-care; gain insight into specific problems and support needs of people with dementia and their carers with a Turkish, Moroccan and Surinamese background; optimizing well-being and quality of life by improving continuity of care, e.g. the transfer to nursing homes.
In this transition experiment, the effects of integrated care are compared with usual care and support. A process analysis identifies factors which enhance the care, by semi-structured interviews and focus groups with key figures at the micro level (social worker, patient and caregiver), meso level (cooperation between care providers / organizations) and macro level. An online record for data-sharing on integrated dementia care is developed. There will be interviews with caregivers, including Turkish, Moroccan and Surinamese carers.
