The project is a longitudinal qualitative study in which 20 health care networks of people with dementia are interviewed three times over transitions that occur in healthcare and the decision making here. This results in a model of SDM in te practice of carenetworks of people with dementia and in the development of a competency and a best practice description on the processsupport of SDM by case managers.
